Just write a polite letter mentioning your appraisals and qualifications and that you would like to be considered for promotion. I wouldn't make any suggestion of wanting to leave etc, unless you have other serious offers of employment, as your employers will know what the job market is like and woudn't take any threat of leaving seriously.

Promotion
I'm trying to write a letter asking to be considered for promotion to 2nd mate. All the online templates seem to be based around meeting sales targets or bringing in clients. Has anyone written one of these recently, if so what did you mention? My last two appraisals have recommended me for promotion and I have all the required certs. just can't put it into words!
