    Whoops....

    So, any thoughts folks?
I love deadlines. I like the whooshing sound they make as they fly by.....

Bit more than just whoops there! It appears that up until about 00:30 they're still operating astern propulsion.. to get that close/drive over those wee boats, I'd hazard a guess that someone wasn't doing much looking out of the window. Over reliance on ECDIS perhaps? Also, going too fast when manoeuvring... Glad I'm not that company's insurer!!

I read a report somewhere, of a similar maneuvering accident where they were using the engine in manual control and the engineer put the engine the wrong way so the captain was ordering ahead but the engines were going astern (although I think it might have been the other way round in the previous accident)

This happens quite frequently ( not always Engineers at fault I must point out) if you go through the MAIB reports you will find "bridge / engine wrong way alarms" not working, contractors not putting the systems back together the right way etc etc

Not all these get reported - at work I have lost count of the times having issues because of the above - the trick is to be aware and to always be on guard!
        Pilotage - It's just a controlled allision

Wouldn't you notice that sort of thing when you do your pre-departure checks? (Assuming you DO the pre-departure checks of course.....)

Oh those things. After 12 months "training" at sea I can reliably inform you that you are meant to tick the boxes and then get the captain to sign it, you only have to check anything if someone from the comany is on board.

This is exactly what I am looking for. Is there an MAIB report for this? Even when some people say they do the checklists, their definition of "do" can sometimes vary.

I would be VERY happy if you filled out my survey .

And I pray to the old gods and the new than I am never in the same waters as you!! It's not a question of slavishly adhering to the checklist, but checking that your propulsions systems and steering gear works before you get going is just plain COMMON SENSE!!

You can never watch too much GoT! Anyway, lets not digress from the topic, this is becoming interesting!

After 12 months training at sea, plus another 12 months working at sea, I can reliably inform you that you are in fact, supposed to do the checks properly! Irrespective of how anyone else does it on your ship, I would get in the habit of doing things the way you are genuinely supposed to, as when you go to another ship it will be expected that you can do the job of an officer competently, which means doing it right.

Perhaps a control failure, I've twice had several experiences of failed wing controls (on open bridge wing vessels) after North Atlantic voyages, not particularly unexpected. Looks very odd and uncontrolled.

It was meant as a tongue in cheek comment, I do know you are actually meant to check the things on the checklist, but that is genuinely what I was taught to do by the officers on more than one ship.

I've been on ships where almost everything I saw was an example of "What not to do". It's difficult to make tounge in cheek comments on a forum, as you can't read tone of voice
