NC - Aye, nailed it. Also, you can get them when you don't meet specific legal requirements. I'll send you a PM of an example.
Obs - Not really as what you're saying is subjective. For example, tiles in the hospital being a bit knackered. It's not a Major NC as it's no one or the environment is at risk, it's not an N/C as there is no specific requirement for them, therefore it's just an Obs saying "they're a bit knackered".
Maybe YM can try and explain what i've said a wee bit better...
