ISM: Observations, Non-conformities, Major Non-conformities

Quick question, want to make sure I've got the right thing in my head here.

So, ISM says:

Struggling a bit to find an example even though I've read observations before. I'm thinking it's more along the lines of "SMS requires crew to wear hard hats on mooring deck but doesn't mention safety shoes. Safety shoes would be a sensible addition." but I'm not sure.

For me, I'd say: -

Major NC - Cheerfully sailing around with a problem on board that could seriously impact on the welfare of those on board or the environment. A broken OWS Sensor or ODME with no dispensation would be a good example.

NC - Aye, nailed it. Also, you can get them when you don't meet specific legal requirements. I'll send you a PM of an example.

Obs - Not really as what you're saying is subjective. For example, tiles in the hospital being a bit knackered. It's not a Major NC as it's no one or the environment is at risk, it's not an N/C as there is no specific requirement for them, therefore it's just an Obs saying "they're a bit knackered".

So, for example, sailing with a manning level below that on the safe manning document, without an exemption?

"Non-conformity" means an observed situation where objective evidence indicates the non-fulfilment of a specified requirement.

"Observation" means a statement of fact made during a safety management audit and substantiated by objective evidence.
