Discharge Book Application

Hi all,

I have been sent discharge book application forms by my sponsoring company, but am having trouble getting the section signed to verify my details, by someone in standing as shown. As its a while since leaving school, the teachers have all changed, I don't know any law etc figures, doctors have not known me for 2 years, & so on.

Is it possible to send your passport with the application instead??? As surely that verifies your identity??

I think you can if I read between the lines right, but not sure.

You'd need to talk to the Registry of Shipping and Seamen for a concrete answer. Give them a call, they're very friendly. I think sometimes companies or colleges can countersign.

If not, have a good think. Teachers, for example, don't have to be your teachers. Any family friends in any of the listed professions? The list is pretty long.

Yeah, The guy down the stair from me is a teacher, not my teacher but he countersigned mine

Merchant Navy Officers are able to countersign applications so maybe some nice soul on here may be able to help you out!
